Case Management
Working with experienced Case Managers allows us to take a team approach to improving quality of life and self-independence for our clients. Together with our Case Managers we develop a care plan including individual goals and objectives to help meet them, medical care and healthy lifestyle maintenance, and fun and community involvement.
